The data request service requirements:

  1. Requestor/PI/co-PI must be affiliated with DHS and have with a DHS Employee or Contract number (e/c#) and DHS email.
  2. The request must only be research-based.
  3. Identified data sets require IRB approval

4. DHS research category (*3-5) assigned

Who need to submit the form?
Any researcher who need to use DHS resources (Including: DHS staff/physician’s time and effort;
Access to DHS Patients for study recruitment; DHS consumables and supplies; Fixed cost DHS resources) need to fill in the form.
